Job Description

An Intake Nurse communicates with the facilities, hospitals and doctor’s offices to verify the discharge date of their patient. They work with the Admission and Intake team and staff members to ensure that all data is correct and collected.

Responsibilities

  • Conduct outreach to patients to gather necessary information required to determine eligibility for qualified home health services.
  • Verify patients’ insurance information to confirm eligibility and identify any discrepancies.
  • Accurately document the details of each conversation in the Electronic Medical Record (EMR) system.
  • Coordinate and communicate effectively with case managers and marketers to ensure proper follow-up and continuity of care.

Minimum Qualifications

A.  Education and Training 

  • Registered Nurse or at least a graduate of Bachelor of Science in Nursing 
  • Active New York State RN license required

B.  Skills and Qualifications 

  • Excellent Communication Skills
    Strong verbal and written communication abilities, with a professional demeanor in interactions with patients and healthcare personnel. 
  • Experience Collaborating with Medical Professionals
    Skilled in working and communicating with physicians, nurses, and allied healthcare personnel in clinical or administrative settings. 
  • Tech-Savvy
    Proficient in using electronic health records (EHR), healthcare management software, and other digital tools. 
  • Insurance Verification Experience
    Knowledgeable in verifying patient insurance coverage, identifying eligibility, and resolving discrepancies. 
  • Strong background to Medical Terminology 
    Confident and knowledgeable in medical terminology to support accurate documentation and effective communication. 
  • Flexible and Able to Multitask
    Capable of managing multiple responsibilities efficiently in a fast-paced and dynamic work environment. 
  • HIPAA Compliance
    Fully aware of HIPAA regulations and committed to maintaining the confidentiality and security of patient health information.
