Job Description

The Provider Verification Coordinator is responsible for obtaining physician information related to patient referrals received from nursing homes, hospitals, and other healthcare facilities. The role involves contacting medical offices, verifying patient care information, coordinating with stakeholders, and escalating unresolved cases when necessary. The coordinator ensures documentation requests are completed accurately and in a timely manner to support patient care and organizational requirements.

Responsibilities

Review referral requests for patients coming from nursing homes, hospitals, and other healthcare facilities.
Contact physician (MD) offices to verify patient care information and request necessary documentation.
Determine whether the identified physician is willing to sign required orders.
Verify if the patient’s last or next physician visit occurred within the last 30 days.
Obtain and document:
o Physician name
o Contact number
o Date the patient was last seen
Resolve tickets when sufficient documentation and provider information are obtained.
Escalate unresolved cases to designated management personnel when patient information cannot be verified.
Maintain accurate records of all communications, follow-ups, and ticket dispositions.
Ensure compliance with healthcare documentation standards and organizational policies

Minimum Qualifications

A. Education and Training

➢ Good communication skills
➢ Tech savvy
➢ Has keen attention to details
B. Skills and Qualifications
➢ Excellent Communication Skills
➢ Have experience in working/speaking with doctors or medical allied personnel
➢ Tech- savvy
➢ Medical background is a plus
➢ Flexible and can multitask
